Raleigh Eye Examinations

Before your Eye Examination

Please call our office to schedule a time for your eye examination. We accomodate walk ins whenever possible, but to be sure, a prebooked appointment is best. If you will be using any vision insurance, please have your card ready. Since verification of benefits occurs before your eye examination, you will be asked to provide your member number, date of birth and/or social security number at time of scheduling.

Approximately one day prior to your eye examination, expect a call or message confirming the appointment. We can give you directions at this time to our office in Raleigh near Triangle Town Center. If you are unable to keep your appointment, please let us know as soon as possible, and we can reschedule.

We recommend printing, completing, and bringing the following office form with you to your eye examination. If you need to locate any of the information, this gives you the opportunity to do so before your arrival. Please be sure to complete your medical history questionnaire in BLACK ink. In addition, please bring your insurance card (if applicable) or other medical history that may be applicable.
