Providing Raleigh Eye Exams
So that we may provide the most complete and comprehensive care, we may ask for your previous eye examination records. If available, please bring any records to your appointment, or provide us with your previous doctors contact information and we will request your records for you.
If you take medications, please bring a list of your medications so we can add them to your medical history.
If you wear contact lenses, please bring the old boxes or lens packages so we can note your current prescription in your record. This will allow doctor to better address any concerns you may have and provide the best lens for your individual needs. If you do not have any boxes or lenses remaining, or you are a new wearer, the doctor will evaluate your eye health and visual requirements to determine the best lens for you.
Diabetic patients should expect to have their eyes dilated as part of their comprehensive evaluation. Please plan accordingly and bring a driver to your appointment if needed. We will be happy to send a summary of your evaluation to your primary care doctor or endocrinologist at your request. Please bring the name and address of this practitioner to your examination.
